At ReliantCare Hospice, we understand that deciding when to initiate hospice care for your patients or loved ones can be a challenging task. Many families express regret that they didn't learn about hospice care sooner. That's why we recommend referring patients for hospice care as soon as possible after a terminal diagnosis. This allows our care team to adequately address the patient's physical, emotional, and spiritual needs.

 

Once a referral is made to ReliantCare Hospice, one of our team members will meet with the patient and their loved ones to explain available programs and services and answer any questions they may have about hospice care. With the patient's consent, our nurse will then assess the patient's needs and create a customized care plan to meet those needs. We work closely with the patient's physician to ensure effective communication and a seamless transition to hospice that maintains continuity of care.
